Little League shoulder is a stress injury to the proximal humeral physis, the growth plate at the top of the upper arm bone, caused by repetitive throwing stress before that plate has closed. It occurs most often in pitchers roughly 11 to 16 years old, particularly during active growth periods, and appears on imaging as a widening of the growth plate compared with the non-throwing arm. The VeloRESET resource distinguishes it from a rotator cuff tear or labrum injury, noting that in growing athletes the growth plate is structurally weaker than surrounding tendons and tends to show stress first.
The guide also addresses the three-month rest period physicians commonly prescribe, characterizing it as a minimum stabilization window rather than a return date. Timelines generally run 8 to 16 weeks before throwing resumes, varying with age, growth stage, and how long the injury developed before diagnosis.
"A follow-up X-ray showing the growth plate has stabilized is medical clearance," said Joey Myers, founder of VeloRESET. "Whether the arm has rebuilt the tolerance to handle a throwing ramp is a separate question, and it gets answered during the ramp, not on a calendar."
For the return-to-throwing period, the guide outlines three tracking questions: whether soreness resolves within 24 to 48 hours after a session, whether the arm feels the same before each session as before the previous one, and whether the week-over-week trend is improving, holding flat, or regressing.
